Management of Keritinized Tissue with Dental Implants
Case Study 10
CASE DESCRIPTION:
This life-changing full-mouth rehabilitation involved a patient who, due to chronic medical conditions, had been unable to maintain adequate oral hygiene over an extended period. As a result, both the upper and lower dentitions had deteriorated significantly, leading to functional limitations, discomfort, and a marked decline in aesthetics. The complexity of the case required a carefully structured and patient-specific treatment plan, with a strong emphasis on long-term maintainability and overall oral health.
After a comprehensive assessment, Dr. Nik designed a prosthetically driven solution centred around implant-supported locator overdentures for both arches. This approach was selected for its excellent balance between function, stability, and hygiene accessibility. One of the key advantages of locator-retained overdentures is their superior cleansability, allowing the patient to maintain oral hygiene more effectively. In addition, the enhanced retention provided by the locator system improves prosthesis stability, while the design offers optimal support for the lips and facial profile, contributing to a more natural and rejuvenated appearance.
With precise surgical execution, Dr. Nik placed four BioHorizons Camlog implants in each jaw, ensuring ideal distribution and positioning to support the overdentures. An immediate provisional phase was initiated, providing the patient with temporary dentures that restored basic function and aesthetics during the healing period. This immediate transition significantly improved patient comfort and confidence from the outset of treatment.
A critical component of this case was the management of the peri-implant soft tissues. During the implant exposure phase, Dr. Nik performed a carefully executed soft tissue procedure involving apical repositioning of a split-thickness flap. This technique was instrumental in developing a stable band of keratinised tissue around the implants, which is essential for long-term tissue health, improved hygiene tolerance, and the prevention of peri-implant complications.
Following successful healing and tissue maturation, the case progressed to the definitive restorative phase. The final implant-retained overdentures were expertly designed by Gosia Ciepiela, a certified dental technician, ensuring an accurate fit, optimal occlusion, and a natural aesthetic outcome tailored to the patient’s anatomy. The prostheses provided excellent retention, comfort, and functionality, while maintaining ease of removal for hygiene purposes.
The final result represented a significant transformation, both functionally and aesthetically. The patient regained the ability to eat, speak, and smile with confidence, while also benefiting from a restoration that is practical and easy to maintain long-term.
This case highlights Dr. Nik’s expertise in full-mouth rehabilitation, implant placement, advanced soft tissue management, and the delivery of highly functional and aesthetic overdenture solutions. His ability to manage complex medical and dental challenges with a tailored, patient-focused approach makes him a trusted referral partner and an excellent mentor for clinicians seeking to advance their skills in implant overdentures and comprehensive restorative care.
