CASE DESCRIPTION:

The patient presented with a failed implant in the aesthetic zone, primarily due to improper buccal positioning and associated facial bone loss. This resulted in both functional concerns and significant aesthetic compromise, requiring a highly controlled and staged regenerative approach. Following a thorough assessment, Dr. Nik carefully removed the failing implant, revealing combined vertical and horizontal bone defects that necessitated advanced reconstruction prior to re-implantation.

To rebuild the deficient ridge, Dr. Nik implemented the well-documented Sausage Technique, as described by Dr. Istvan Urban, to achieve predictable bone regeneration. During the first surgical phase, guided bone regeneration was performed using a titanium-reinforced, non-resorbable dense PTFE membrane to maintain space and stability. This was combined with a carefully prepared mixture of autogenous bone particles and MinerOss allograft from BioHorizons Camlog UK, promoting both biological activity and volumetric support. The site was managed meticulously to ensure optimal healing conditions.

After a successful and uneventful healing period of seven months, sufficient bone volume had been regenerated to allow for implant placement. Dr. Nik then placed a Conelog Progressive implant in an ideal prosthetically driven position. To further enhance stability and contour, additional grafting was carried out using a resorbable striated collagen membrane in conjunction with the Mini Sausage Technique, reinforcing the regenerated architecture and supporting long-term outcomes.

Recognising the importance of soft tissue aesthetics in the anterior region, Dr. Nik also performed a connective tissue graft harvested from the palate.
