Full Arch Rehabilitation

Case Study 7

CASE DESCRIPTION:

This life-changing full-mouth rehabilitation involved a patient experiencing long-standing, complex dental issues that had significantly impacted both function and quality of life. The extent of deterioration required a highly structured and comprehensive treatment plan, with careful consideration given to aesthetics, occlusion, and long-term stability. Working in close collaboration with Gosia Ciepiela, a certified dental technician, Dr. Nik developed a fully customised approach to redesign the patient’s smile, optimise occlusal harmony, and re-establish an appropriate vertical dimension.

A key aspect of the planning phase focused on restoring the occlusal vertical dimension (OVD), which had been compromised over time due to wear and tooth loss. Through detailed analysis and digital planning, the new OVD and occlusal scheme were carefully established to improve facial proportions, functional efficiency, and overall patient comfort. This interdisciplinary approach ensured that both surgical and prosthetic phases were seamlessly aligned.

Following the strategic removal of the remaining compromised teeth, Dr. Nik proceeded with immediate implant placement, positioning the implants in prosthetically driven locations to support a full-arch restoration. An immediate loading protocol was implemented, allowing the patient to receive fixed provisional restorations on the same day. This not only restored function and aesthetics instantly but also provided significant psychological and physical comfort during the healing phase.

Over the following six months, the implants integrated successfully, with stable hard and soft tissue conditions achieved. Throughout this period, the provisional restorations played a crucial role in guiding soft tissue contours and allowing for functional adaptation to the new occlusal scheme and vertical dimension.

Once healing was complete, Dr. Nik transitioned to the definitive restorative phase, delivering final hybrid restorations designed according to the FP3 concept. This design approach allowed for optimal replacement of both teeth and supporting structures, while effectively increasing the occlusal vertical dimension and enhancing occlusal balance. The final prostheses provided a harmonious blend of strength, durability, and natural aesthetics, tailored specifically to the patient’s anatomical and functional needs.

Before After

The end result was a dramatic transformation, with the patient achieving a stable, functional, and highly aesthetic smile. Improvements were evident not only in dental health and appearance but also in overall confidence and quality of life.

This case highlights Dr. Nik’s expertise in complex full-mouth rehabilitation, immediate loading implant protocols, and advanced occlusal reconstruction. His collaborative approach with highly skilled technicians, combined with meticulous planning and execution, makes him a trusted referral partner and an excellent mentor for clinicians seeking to advance their skills in comprehensive implant and restorative dentistry.
