CASE DESCRIPTION:

This case involved a complex bone regeneration procedure in the posterior mandible, where insufficient bone volume presented a significant challenge for future implant placement. In order to create a stable and long-lasting foundation, a highly controlled and biologically driven approach was required. Dr. Nik selected the advanced Khoury Plate technique, recognised for its predictability in restoring both horizontal and vertical ridge deficiencies using autogenous bone.

A cortical bone block was carefully harvested from the mandibular ramus using piezosurgery, a technique that allows for exceptional precision while minimising trauma to surrounding tissues. The bone block was then meticulously processed into a thin cortical plate, alongside autogenous bone particles, maximising the regenerative potential of the patient’s own tissue.

The cortical plate was precisely adapted to the recipient site and stabilised using mini fixation screws to recreate the natural contour of the ridge and maintain space for regeneration. The autogenous bone particles were strategically placed within the defect, supporting vascularisation and new bone formation. This combination of rigid stabilisation and biologically active graft material provided an ideal environment for predictable healing and volumetric restoration.

Following a healing period of four months, which progressed smoothly and without complications, the previously deficient ridge had been successfully regenerated to near-original volume. The restored bone architecture provided a solid and reliable base for implant placement, demonstrating the effectiveness of the technique when executed with precision.

To ensure optimal implant positioning, Dr. Nik utilised a surgical guide, allowing for accurate, prosthetically driven placement. A Conelog Progressive implant with a 4.5mm diameter from BioHorizons was inserted in the ideal three-dimensional position, achieving excellent primary stability and alignment to support long-term functional and restorative success.
